Physicians - Follow These Easy Steps
- When you have a patient requiring a device, fill out our online enrollment form and fax or email (online enrollment) to us. A photocopy of the insurance information along with a copy of the insurance card should be faxed with the enrollment form.
- If you do not keep a monitor in your office, simply fax the information to us and we will "overnight" the monitor directly to the patient or your office.
- If you have a monitor in your office, get a baseline recording and call us. We will instruct the patient further if needed. If you're busy, simply give the monitor to the patient. We will call, give instructions and get a baseline once they are home. Remember to include the patient's home telephone number on the faxed enrollment form.
- The patient may keep the monitor for up to 30 days, symptomatic arrhythmias are usually recorded within the first two weeks. Every time your patients call the monitoring center to transmit, we send the results to you. At the end of the monitoring period, the patient returns the monitor to you or to us and we will send you a final report with all the ECG strips that were transmitted.

They will be able to call us with any questions they may have regardless of the time of day or whether they have a recording to transmit.
